Survivor Referral Form

All information will be kept confidential and on file for 5 years in accordance with the Australian Privacy Act. By signing this form, you consent to management at the Freedom Hub being made aware of the information to best meet the client’s needs. Personal information will not be shared outside of the organisation unless authorised by the individual, except for the following situations (in the event of an emergency or there is an immediate risk to self or others). The individual can request access to, or correction of, their file notes and personal records via charityadmin@thefreedomhub.org. Occasionally, the court can subpoena paperwork and legal proceedings; in this case, the individual will be informed immediately.

Please note that the more information you provide here, the less likely the client will need to repeat it to us. This assists in their experience of The Freedom Hub onboarding process.
